Chronic Care Case Manager
Location: Chad Brown Health Center

Description:
Chad Brown Health Center is seeking a Chronic Care Case Manager to coordinate and provide on-site chronic disease case management to Dexter Manor High Rise residents by assisting residents in accessing community-based programs and services.  Will act as a resource between the health center and residents in coordination of care.  Responsible for maintaining health promotion and patient education materials and resources regarding management of chronic diseases, i.e., hypertension, diabetes, asthma, and chronic kidney disease. Registered Nurse

Behavioral Health Clinician

Social Services Case Manager

Description: Provide case management services, financial resource counseling, crisis intervention, supportive counseling, pregnancy testing and family planning counseling to high-risk health center patients. Medical Receptionist Supervisor

Description:

The duties of this position include (but are not limited to):  To provide supervision to the team of Medical Receptionists.  To participate as a member of the care team.  To perform basic duties related to overall patient services at the Tri-Town Community Action Agency.  To assist patients in a timely manner, schedule appointments and respect confidentiality of the Agency and patients.  To ensure efficient patient flow.  To develop and maintain effective staffing schedules.  To provide orientation to the electronic practice management system, patient registration functions, and other patient registration activities to new staff members.  T provide coaching to all front desk staff members.  To design, implement and evaluate performance improvement functions related to front desk operations.  To monitor and ensure effective patient payment processes.  To handle patient concerns related to reception operations (including scheduling conflicts).  To work with the remainder of the care team (particularly Nurses, Medical Assistants and Primary Care Providers) to ensure appropriate patient flows (e.g. informing patients if providers are running behind, managing late patients. etc).  To complete appropriate documentation of phone messages/calls in the electronic practice management system and electronic medical record system.  To greet patients upon their arrival, ensuring that they complete the initial paperwork.  To register and accurately update patient’s information in the electronic practice management system.  To verify insurance or sliding scale fee and obtain pre-authorization when necessary; documenting the results of these authorizations or verifications in the electronic practice management system.  To conduct patient orientation sessions for new health center patients and distribute patient orientation manuals.  To participate in the Performance Improvement Committee Meetings and or other committees when requested by the Health Center Director or Health Disparities and Patient Education Coordinator.
